What the application collects
Who is completing the application: an adult for themselves, or a parent or guardian for a minor
The client's full name, a contact email and number, and city and country
The guardian's name and relationship, when a guardian applies
The main goal, training experience and background, and the biggest obstacle
The preferred coaching setup, weekly availability and time zone, equipment and training space, and preferred starting period
Consent to be contacted
The application does not ask for payment details, signatures, medical records, or medical clearance.
How it is used
I use your answers to review your application, contact you about coaching, and prepare for your 30-minute discovery call. No payment is collected through the application form.
Where it is stored
Each submitted form is stored in the Restart Fitness PH site database on Wix (Wix.com), with a reference number and the time it was received. Form answers are not saved in your browser.
Form records are kept for one year. To request access to or deletion of your form, send a direct message to @restartfitnessph on Instagram and include your form reference.
